GPs operate as independent professionals and as such are entitled to make a charge for work which falls outside the work which the government contracts them to do for the NHS.
In general private medicals, and a variety of paperwork including private sick notes, insurance forms etc are not paid for by the NHS, and so will carry a Doctor's fee.
This will sometimes be reimbursed by the organisation which requested the report, and other times the patient will be liable. You will be advised of any fees incurred prior to any non NHS work being carried out.

Any simple private medical insurance claim forms. Claim forms, including letters of confirmation or other forms of certification
Holiday cancellation certificates including written confirmations
Full Private medical examinations and report: Includes (but is not limited to) Large Goods Vehciles (LGV), Taxi, Pilot, Employment medicals, council reports, Elderly, diabetic certification, seat belt exemptions and fitness to travel examinations
Copies and printouts of records per A4 sheet

Non NHS vaccines e.g. rabies, meningitis C (travel), Hepatitis B (non NHS), Japanese Encephalitis
Cost of injection. This is paid to chemist on collection of vaccines, (except) Hepatitis B which is £15.00 and paid to the surgery plus £30 injection administration charge to The Garth
Lasting Power of Attorney
